Description
Complete the following steps to start the HSI Instructor process:
- Complete the HSI Application for Authorization (form must be completed by new and renewing instructors.)
- Send completed form, along with all valid, professional certificates that have been checked off in Steps # 3 and 4 on the HSI Application for Authorization, to LifeForce USA, via email, fax or mail.

Fax #: 732-919-6090To Reauthorize as an HSI Instructor, you must have taught at least one HSI classes during the previous two-year authorization period. If you have not met the teaching requirements, you must provide proof of current equivalent instructor certification. If you have met the teaching requirements, just follow Steps # 1 and 2 listed above.
